Insurance & Payment Methods
Saltz Plastic Surgery & Spa Vitória is a cash-pay practice. We do not work with insurance providers, Medicare, or other third-party payors, and we do not provide documentation for patients to submit to insurance.
We accept:
- All major credit and debit cards
- Wire transfers
- Patient financing (CareCredit & PatientFi)
- Health Savings Account (HSA) funds (when applicable)
- Cash
Other Policies
Gift Cards & Retail
- Valid for one year; redeemable only for Spa Vitória treatments and products
- Cannot be applied to surgical consultations, fees, or gratuity
- Retail items may be returned within one week for Spa Vitória credit only and must be unopened (no refunds)
Surgical Estimates & Payments
- Consultation fees are non-refundable (credited if surgery is scheduled)
- Estimates include pre- and post-op appointments. Lab work, pathology, prescriptions, and garments are separate
- $1,000 non-refundable deposit required to schedule surgery; applied to your balance
- Full payment due at your pre-op appointment, four weeks before surgery at or before your pre-op appoitnment.Â
- Rescheduling = $1,000 fee; canceling within four weeks forfeits fees
- Quotes valid for three months
- Third-party facility and anesthesia fees may vary
Cancellation & No-Shows
- 24-hour cancellation notice required (Cancellations for Monday appointments must be received on Friday)
- $150 fee for missed surgical consultations, wellness, injectable, or spa visits
- Credit card required on file
- Late arrivals may shorten your visit
Holladay Office Procedures
- Deposits set individually for in-office procedures
- Sedation fees paid directly to the anesthesiologist (cash or check)
- Surgery times confirmed the morning of your procedure
Revisions & Additional Treatments
- Results vary; revisions may require additional surgeon, anesthesia, or facility fees
- Spa Vitória treatments may be recommended to enhance results (not included in surgical estimates)
